QTP crashes when script is run or for any reason

You have few options to try:
1. Uncheck the 'Save movie to results' option under Tools>Options>Run:Screen Capture, in case it is checked. This may be one quick reason.
2. Check for addin issues:
Disable all addins (one by one) and check if issue caused by any particular addin
ActiveX addin is known to cause such issues
In such case some registry keys need to be reset
3. If that does not work
Check for patch issues
Uninstall all patches (one by one) and check if issue is caused by any particular patch
WEB_QTP_0078 patch is known to cause such issues
4. Navigate to c:\windows\SysWOW64\PKG_LOGS
Look for log file related to qtp “Hewlett Packard Quick Sh**t” something like that
Read the log and check for any dll initialization issues
Un-register/Re-register that particular dll (if found in logs)
5. If that does not work, try system restore
Start>type 'system restore'
Follow the wizard to complete it.
